The United States Citizenship and Immigration Services (USCIS) requires a medical and physical exam as part of the residency and/or entry process to the United States. The rules and requirements are designated by the USCIS:

Generally, allapplicants filing for adjustment of status to that of a lawful permanent resident must submit Form I-693 completed by a designated civil surgeon. Form I-693 is used to report results of a medical examination to USCIS. The examination is required to establish that an applicant is not inadmissible to the United States on public health grounds.

Prepare for the I-693 Exam

To speed up the process, please provide all historical vaccination and immunization records that you may have to Dr. Atalla

       Please review the items below:

  1. Bring your passport, driver’s license, or any other government-issued ID. Ensure all documents are unexpired.
  2. Make sure you bring your Alien number #. If you are not sure, ask your attorney.
  3. Bring your entire vaccination record.
  4. If you recently got a flu shot, kindly bring those records.
  5. We have the USCIS I-693 forms at the office for your convenience.
